POSITION SUMMARY:

Under the direction of the Referrals Supervisor, this position is responsible for accurate and timely data entry of referral requests into the appropriate portal, follow up on all specialty referrals, works as a liaison between the patients, providers, and IPAs to ensure timely access to specialty care.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Referrals entry and processing based on assigned providers or department needs to ensure timely submission, patient communication, and addressing of all referrals/authorizations within the required timeframe based on urgency.
  • Timely and accurate submission of referrals/authorizations into the appropriate portals.
  • Verify financial eligibility prior to submission, follow procedures for uninsured patients, and process in-house referrals available for our patients.
  • Follow up on pending referrals/authorization for timely communication of approval and specialty care information to patients.
  • Notify patients of approved referrals/authorizations, and documentation of outcomes in EMR system.
  • Assists patients with scheduling of their specialty appointments, document outcomes, follows up with the specialist office to request reports/consult/visit notes, and close the loop after the appointment.
  • Completes intake forms/packets/additional forms for specialty offices to expedite scheduling of specialty appointments.
  • Communicate and collaborate with providers, clinical and non-clinical support staff, and other departments on updates pertaining to referral submission/follow up to ensure accuracy and completeness of all requested referrals/authorizations.
  • Promote a positive working relationship with community agencies, external and internal stakeholders.
  • Participate in the daily/weekly huddle, and monthly department meeting led by the Referral Supervisor and provide feedback on updates.

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Highschool graduate or equivalent.
  • Knowledge in medical terminology, and computer proficiency required, EMR user experience and experience in community clinic are highly desirable.
  • Excellent organizational skills, oral and written communication skills.
  • Bilingual English/Spanish required.
  • Ability to work with population of diverse cultural, educational, social, and economic backgrounds.
