Mission City Community Network (MCCN) is seeking a compassionate and skilled Physical Therapist to join our multidisciplinary healthcare team in North Hills. The Physical Therapist will provide comprehensive evaluations, treatment planning, and rehabilitation services to patients with a variety of musculoskeletal, neurological, and functional conditions. This position offers the opportunity to make a meaningful impact in a community-focused healthcare environment while working alongside dedicated providers and support staff.

Responsibilities

• Supervise and direct Physical Therapy Assistants
• Perform comprehensive physical therapy evaluations and assessments.
• Develop individualized treatment plans and measurable rehabilitation goals.
• Provide evidence-based therapeutic interventions, including therapeutic exercise, manual therapy, neuromuscular re-education, gait training, balance training, and functional mobility training.
• Monitor patient progress and modify treatment plans as appropriate.
• Educate patients and caregivers regarding treatment plans, home exercise programs, wellness strategies, and injury prevention.
• Collaborate with physicians and interdisciplinary team members to coordinate patient care.
• Maintain accurate and timely documentation in the electronic medical record.
• Participate in quality improvement and patient care initiatives.

Qualifications

• Doctor of Physical Therapy (DPT) degree from an accredited program.
• Current, unrestricted California Physical Therapist License.
• Current BLS/CPR certification.
• Minimum two years of clinical experience preferred.
• Strong communication and interpersonal skills.
• Bilingual English/Spanish preferred.
• Experience with electronic health records (EHR) preferred.
• Ability to work effectively with diverse patient populations.
